Peer Support Worker: CCSS/CCSW credential in good standing in the state of NM.


We are seeking passionate Peer Support workers to join our team. The Comprehensive Community Support Worker is responsible for providing individuals with services and resources necessary to promote recovery, rehabilitation, and resiliency. CCSW addresses goals specifically in the following areas: independent living, learning, working, socializing, and recreation. Comprehensive community support services consist of a variety of interventions based on coaching and addressing barriers that impede the development of the necessary skills for independent functioning in the community.


Key Responsibilities: 

  • Identify client needs and facilitate connections with appropriate resources within CRP and within the community.
  • Provide case management and referral services for client caseload.
  • Assess, plan, implement, and evaluate the needs of clients to develop and facilitate an appropriate plan of care and a sustainable and successful discharge plan for clients to live their best life.
  • Work closely with clients to help identify healthy resources in the community, friends, and family to plan, secure, coordinate, and monitor services via community and informal resources to assist in reaching optimal social, psychological, and physical functioning towards recovery and resiliency goals.
  • Complete all necessary documentation required in the electronic health record within 24 hours of services provided.
  • Attends all meetings and supervision specified as mandatory by either the direct supervisor or see CRP executive leadership.
  • Completes the required training curriculum and ongoing training to improve knowledge skills and abilities to promote awareness and appreciation of cultural diversity and needs of persons served.
  • Supports development and opening of additional clinical offices.
  • Additional duties as assigned.

Job requirements

  • Certified Comprehensive Community Support Worker
  • Current and valid NM Driver License with an acceptable driving record (as defined in the guidelines)
