Join Our Team as a Certified Peer Support Worker (CPSW)!

What You’ll Do:

  • Identify client needs and facilitate connections with appropriate resources within CRP and within the community.

  • Provide case management and referral services for client caseload.

  • Assess, plan, implement, and evaluate the needs of clients to develop and facilitate an appropriate plan of care and a sustainable and successful discharge plan for clients to live their best life.

  • Work closely with clients to help identify healthy resources in the community, friends, and family to plan, secure, coordinate, and monitor services via community and informal resources to assist in reaching optimal social, psychological, and physical functioning towards recovery and resiliency goals.

  • Complete all necessary documentation required in the electronic health record within 24 hours of services provided.

  • Assist with facilitating IOP Group therapy sessions.

  • Attends all meetings and supervision specified as mandatory by either the direct supervisor or see CRP executive leadership.

  • Completes the required training curriculum and ongoing training to improve knowledge skills and abilities to promote awareness and appreciation of cultural diversity and needs of persons served

Job requirements

  • Must hold a valid CPSW certification

  • Clean Driving Record and Valid NM State drivers license.

  • Experience working with individuals who have mental illness, and substance use disorders.
